Hi Liz,
The “skin change” as it’s called, is more of a mental, telekinetic ability than anything else. Dragons don’t physically change form. Their minds create a sort of energy field around them that reshapes the perception of their appearance, both for them and anyone who sees them. Their eyes tell them they look like humans, their nerves tell them that their skin is that of a human’s and it’s the same thing or humans who see or interact with them. The ability manifests around physical maturity and maintaining the form usually becomes autonomic with a little practice.
So essentially it’s a chameleon-like illusion, and there are limitations. For starters, they can’t appear as something much larger or smaller than their actual mass. What’s more, their hair and eyes do not typically change color. Those remain the same.
To your second question, why wasn’t this used during the Red War? I covered this in a little bit of detail early on, but essentially, it was considered a lost art. What’s more, it wouldn’t have been practical given the immense size of early generation dragons. In a now-scrapped book, I actually went into a lot more detail about how Qira rediscovered the ability soon after her mind merged with Radley’s before he died. This lost art that allowed her to “skin change” as you called it, became an evolutionary trait as, later generation Terran-born dragons have more advanced mental abilities than most of the first gen, and actual elder Drakeas (excepting Tar’Mortha, of course, but she’s a unique case.)
